CHOOSING A NAME
OPTION 1: The professional entity name is the practitioner’s name, followed by the profession that will be practiced, and ending with the corporate indicator. For example, John Smith, Family Health Nurse Practitioner, P.C.
OPTION 2: The professional entity name is a unique name or phrase, followed by the profession that will be practiced, and ending with the corporate indicator. For example, Care Connect Family Health Nurse Practitioner, P.C.
HOURLY RATE BILLING
If the initial name that is submitted for consent is rejected, all work done to revise and resubmit the proposed name will be billed at our hourly rate.
*Includes 1 proposed name submission to the Office of Professions for consent. All proceeding submissions will be billed at an hourly rate.
